Circular copper medal with eyelet for ribbon suspension; the face with monumental composition ‘Takovo uprising’ by the sculptor Petar Ubavkic representing the Serbian leaders Milos Obrenovic and Melentije Pavlovic, inscribed ‘TAKOVO 1815’ to the left; the reverse with the Takovo Memorial to the Uprising erected in 1887, circumscribed in Cyrillic characters ‘OVAJ GRM VREME CE DA OSUSI I KAMENI STUB CE DA PORUSI, AL’ SRBIJA VECITO STAJACE I MILOSA IME SPOMINJACE’ (This bush will break down the stone pillar but Serbia will stand forever and the name of Milosha will be remembered), a reference to the Takovo oak under which the Uprising was declared and to Milos Obrenovic; on original ribbon with miniature device on the ribbon identical to the face of the medal.

The First Serbian Uprising of 1804 against the Ottoman Empire came to an end in 1813 and most of its leaders fled to Austria but Milos Obrenovic remained behind and led the Second Uprising which began at Takovo on 23 April 1815 and led to autonomy for the Serbs later that year with Obrenovic created Prince of Serbia.  Serbia eventually gained full independence under the Treaty of Berlin in 1878.
